McDonald's Hot Fudge Sundae Attack - Sundae was served with hot fudge on the bottom.

Knoxville, Tenn. resident James Davis Wilson went into a rage in February of 2010 when the sundae he ordered at McDonald's was served with hot fudge on the bottom. He later punched an employee in the face in an argument stemming from the perceived injustice.

McDonald's Customer Dies At Counter In Restaurant Staff Reportedly Keeps Serving Food

A McDonald's restaurant in Sibenik, Croatia, is drawing international attention today after workers there reportedly continued to make and sell food after a female customer collapsed and died at the counter.

Croatian newspaper 24sata reports that, despite the death of the McDonald's customer, "employees continued to work as if nothing had happened."
